Refer a Patient for Dry AMD / Valeda Evaluation

We welcome referrals of patients with dry age-related macular degeneration for evaluation for Valeda photobiomodulation.

Please send recent examination findings, visual acuity, OCT imaging if available, medication list, and insurance information with the referral. We are happy to evaluate candidacy and communicate our findings and treatment plan back to the referring doctor.
